Badge Holder
The Ethiopian Public Health Association (EPHA) in collaboration with the World Federation of Public Health Associations (WFPHA) will host the 13th World Congress on Public Health from 23 to 27 April 2012 in Addis Ababa, Ethiopia. The main theme of the congress is "Moving towards Global Health Equity:
Opportunity and Threats. Up to 3000 researchers, .academicians, scientists, trainers, programmers, policy makers and students of public health and medicine all over the world will participate in this historic Congress.
1. Ethiopian Public Health Association (EPHA) wishes to invite eligible bidders for the procurement of conference badge holder as per the specification provided below.
* Conference Name Badge Holder Quantity: 3500
Size (W x H): llcm x 15cm (Vertical) Material: Plastic
Usage: Hold name badges for congress delegates and folded program
Additional Features:
-Two poCket holder (top and back)
-With chain boles
-No need of accessories (lanyards, self attached safety pin or hook)
Design and Quality: As per sample available at EPHA office
2.Bidders are required to see the badge sample available at EPHA office (at the front desk) during office hours (08:00am-05:00pm) before submission of the bid document.
3.AII bid documents shall be sealed and inserted in the bid box prepared for this purpose before February 29, 2012, !0:00am at EPHA, front desk, located on the road to "Meskel Flower" near Dream liner Hotel.
4.Bidders are required to submit VAT registration certificate and renewed business license.
S.Bid shall be accompanied by a bid guarantee amount birr 3,000.00 (three thousand birr) in the form of Cash Payment Order (CPO) in the name of Ethiopian Public Health Association.
6.The bid shall be valid for 4S calendar days after bid opening.

Failure to comply with any of the conditions here in above numbers 1 to 8 shall be automatically rejected.
EPHA reserves the right to accept or reject any or all bids.
